Before you come we encourage you to register your kid(s) in CTA Website

This includes both new and existing students.

Note: You will only register your child on the CTA website. Payments will be made to our Tamil School.

Step 2 – Sign

Volunteer Agreement / Student Waiver / Release of Liability

VOLUNTEER AGREEMENT

This Agreement is made and entered into as of this day between (“Volunteer”) and GATS-Lilburn Tamil School (“GATS-LTS”).

In consideration of the mutual covenants and promises of the parties hereto, Volunteer and GATS-LTS agree as follows:

GATS-LTS is a 501(c) (3) nonprofit organization organized to teach Tamil language and values to children of all age groups. Volunteer acknowledges and agrees that he or she (1) freely and voluntarily offers to perform services for GATS-LTS in connection with GATS-LTS’s nonprofit activities, without fee, charge or compensation or expectation of compensation beyond reimbursement for pre-approved expenses, (2) will perform tasks at the direction of GATS-LTS and its staff, and (3) such participation will not, to his or her knowledge, violate or infringe rights or obligations of Volunteer to third parties. The volunteer shall not be considered an employee of GATS-LTS.

Acceptance and Termination: GATS-LTS accepts the service of Volunteers with the understanding that such service is at the sole discretion of GATS-LTS. Acceptance of a Volunteer is subjected to and conditioned on Volunteer’s execution and acceptance of this Agreement and continued adherence to these terms. Volunteer must be officially accepted and enrolled by GATS-LTS prior to performance of any task. Volunteer agrees that GATS-LTS may at any time, for whatever reason decides to terminate Volunteer’s relationship with GATS-LTS.

Rights Reserved: All work product, including without limitation works of authorship, created during or in the course of Volunteer’s service to GATS-LTS and all intellectual property rights thereto, including without limitation copyrights, patent rights, and trademark rights, shall be owned by GATS-LTS. In addition, to the extent any law or treaty prohibits the transfer or assignment of any moral rights or rights of restraint the Volunteer has in the Contribution or the Work, Volunteer waives those rights as to GATS-LTS, its successors, licensees and assigns.

Confidentiality: Volunteer is responsible for maintaining the confidentiality of all proprietary or privileged information to which the Volunteer is exposed while serving as a Volunteer whether the information involves a single staff, volunteer, client or other person or involves overall GATS-LTS business. Volunteer recognizes the value and sensitivity of confidential information and understands that it is protected by law. Volunteer agrees to keep all such information confidential for an indefinite period of time, even after he/she is no longer volunteering with GATS-LTS.

Failure to maintain confidentiality may result in termination of Volunteer’s relationship with GATS-LTS or other corrective actions.

These policies do not constitute, either implicitly or explicitly, a binding contractual personnel agreement. GATS-LTS reserves the exclusive right to change any of these policies at any time and to expect adherence to the changed policy.

In Witness Whereof, the undersigned have executed this Agreement as set forth above.

STUDENT WAIVER AND RELEASE OF LIABILITY

In consideration of being allowed to participate in any way in GATS Lilburn Tamil School related classes, events and activities, the undersigned:

Agree that the parent(s) and/or legal guardian(s) of the participant should inspect the facilities and equipment to be used, and if the parent or guardian believes anything is unsafe, he or she should immediately advise supervisor (advisor, manager, etc.) of such condition(s) and refuse to participate.
Acknowledge and fully understand that each student/participant will not be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the actions, inaction, and negligence of others, or the conditions of the premises or of any equipment used. Further, there may be other risks not known to us or not reasonably foreseeable at this time.
Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
Release, waive, discharge and covenant not to sue Lilburn Tamil School its affiliated organizations, GATS and CTA, their respective administrators, directors, agents, and other employees of the organization, other members/participants, sponsoring agencies, sponsors, advertisers , owners and leasers of premises used to conduct the class/event, including Duluth Middle School all of which are hereinafter referred to as “RELEASES”, from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.

I HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY.
